Appointment Requests    

We request that you schedule appointments in advance. However, same day appointments are available in an emergency. Depending on the situation, our nursing triage may direct you to the appropriate hospital.

At the time of initial appointment please have the following ready: address, social security number, insurance card and phone numbers for home and work. When you schedule a stress test, we will need your weight and medication allergies. Please inform us if you are diabetic.

At the time of appointment, please bring your current medications, insurance card, and obtain any pertinent medical records.
