We participate with a large variety of insurance
and managed care plans. Most insurance companies require
precertification for inpatient hospital stays and some outpatient
services. Managed care plans require precert from your PCP for all
services. It is your responsibility to confirm that
precertification has been obtained from your insurance company, HMO
(Health Maintenance Organization), PPO (Preferred Provider
Organization), or PPC (Preferred Patient Care). Your failure to
obtain precertification may result in rescheduling of your
appointments or denial of your claims. Our insurance representative

Unfortunately, with today's business of medicine, the following list
of insurance plans is in constant change. Due to prior arrangements
made by your insurer, we cannot always see every patient covered by a
specific plan. If you have questions regarding our participation with
